Carers Bucks works in partnership with Buckinghamshire GP practices, to ensure that patients who are carers are identified and well supported.
We encourage carers to let their GP Practices know that they are in a caring role so that they can be added to the GP Practice’s Carers Register.
Our team of adult carer support workers are all attached to a particular Primary Care Network in the county and they are the point of contact for the GP Practices within that specific PCN.
We are also able to deliver carer awareness training sessions for GP Practice staff, within PLT, or at a time of the practice’s choosing. If you are a GP Practice and would like Carers Bucks to deliver a carer awareness training session for Practice staff (usually during PLT dates), please call us on 0300 777 2722 or email us.
