Intermediate care is made up of services that help people stay in their own home (or care home) instead of going into hospital because of illness. It can also help support someone after a hospital stay with the aim of preventing readmission or allowing early discharge. In Bucks, depending on the person’s needs, intermediate care can last from a few days or weeks.
